The complexities of health care require

strong emphasis on maintenance of strong links
with family /whanau, general practice, social
services, hospital and of course the funders such

A new service has been developed in conjunction
with ACC to provide community nurses in southern
DHB covering Southland, Dunedin, Central Otago
and Queenstown from September 2012.
The service starts with an in-depth assessment of
you or your loved one's lifestyle, medical
conditions and the kind of nursing support that is
needed in order to remain independent at home
after an injury.
The purpose of this assessment is to meet with you
and your family and jointly determine the scope of
